PAYMENT:
Payment is due at the time of said class. Invoices may be sent via Square. Payment may be made by credit card, cash, or check made out to Barre None Dance Company. All credit card transactions are subject to a 3% processing fee by Square.
COVID:
The spread of SARS-CoV-2 (the “novel coronavirus”), which causes the disease known as COVID-19, was declared no longer a worldwide pandemic by the Federal COVID-19 PHE declaration on May 11, 2023. Most tools, inclusive of vaccines, treatments and testing have remained available and distributed through the HHS, who remains committed to maximizing continued access to COVID-19 vaccines. The undersigned Member agrees that BNDC cannot prevent the presence of the novel coronavirus and/or of other such significant public health conditions (collectively with the novel coronavirus, a “Public Health Condition”) in and about the locations where BNDC provides instruction (the “Locations”) and cannot prevent any person present in and about these Locations from becoming exposed to, contracting, or spreading the novel coronavirus or any other Public Health Condition.. I HAVE CAREFULLY READ AND FULLY UNDERSTAND ALL PROVISIONS OF THIS RELEASE, AND FREELY AND KNOWINGLY ASSUME THE RISK AND WAIVE MY RIGHTS CONCERNING LIABILITY AS DESCRIBED ABOVE:
RELEASE OF LIABILITY:
Participation in Barre None Dance Company, hereafter referred to as Company, primarily requires moderate to intense movement and physical activity. As with any sport, full participation in Company may involve the risk of physical injury. While it is Company’s intention to provide quality, safe, and manageable physical dance education, we cannot guarantee all participants abstaining from any bodily harm. As such, Company, its instructors, partners, and affiliated businesses are not liable for any injuries incurred by yourself while attending any Company class, rehearsal, performance, and event. Company, its instructors, partners, and affiliated businesses will not be held liable for any medical expenses due to injury incurred by yourself while attending any Company class, rehearsal, performance, and event.
